Pre-Participation Forms

Welcome to Lakeland College! The Athletic Training Staff looks forward to helping you and any of your medical needs that you may have in the future here at LC. Please do not hesitate to contact us if you have any medical problem. We are here to help and provide quality health care to all Muskie Athletes.


All student-athletes who plan on participating (on the roster or trying out) must have completed medical forms and insurance on file in the athletic training facility prior to engaging in intercollegiate athletic event, including strength and conditioning sessions, try-outs, practices, or competitions. The pre-participation Physical Evaluation MUST be done on Lakeland College form, other forms will not be accepted. The pre-participation physical now includes a REQUIRED screening for the Sickle Cell Trait or a signed waiver.


All student-athletes must have acceptable form of insurance that covers intercollegiate athletics. The NCAA now requires insurance coverage of $90,000. Copy of insurance card must be uploaded or sent to Lakeland College Sports Medicine.

FRESHMEN/TRANSFER STUDENT-ATHLETES

All athletes NEW to Lakeland College or returning after a leave from competition must complete a 1) medical history questionnaire, 2) physical exam and 3) insurance form prior to participation (practice or event) in intercollegiate athletes.

RETURNERS

All returning athletes must 1) complete an annual physical exam, 2) update their medical history, and 3) update or change their insurance information prior to participation (practice or event) in intercollegiate athletics at Lakeland College.

At check in additional forms will be initialed if no changes were required.

ADHD INFORMATION

If a student-athlete has been diagnosed with Attention Deficit Hyperactivity Disorder (ADHD), this form should be given to the diagnosing physician in order to fulfill the NCAA's requirement concerning the medical support for the student-athlete begin treated for ADHA with a stimulant medication.

SICKLE CELL TRAIT INFORMATION

Screening for sickle cell trait as part of the medical examination process is required for all NCAA student-athletes unless documented results of a prior test are provided to the institution. INTERCOLLEGIATE HEALTH INSURANCE REQUIREMENTS

As an active NCAA Division III institution we must certify that all of our student-athletes secure sufficient insurance coverage for medical expenses resulting from athletically related injuries sustained by a student-athlete participating in the covered event in an intercollegiate sport as recognized by the participating institution.

In order to work better with your insurance company please provide preferred local area hospitals and clinics that your insurance policy will cover you under. The list of local area hospitals and clinics is listed below. If your insurance does not cover these locations, search nearby areas that your insurance will cover.
