Community Service Form:

Is your organization volunteering within the community? If so, we want to know about it! Please complete the following information so that we can track your service hours. We appreciate your efforts to better our community.

Registration Form:

All student organization must be registered through the Student Activities office in order to receive the services provided by the college. Each organization must re-new their membership every year. Please re-submit the officer information form EVERY time new officers are elected.

Pub Request Form:

If your organization wants to host an event in the Lakeland College Pub, you must submit this form at least two weeks prior to the proposed event. Please give as much information as possible regarding the event so the Pub staff can be adequately prepared. Please submit this form to Talia Proffitt, Director of Student Activities in Laun 100 or Tim Romanello, Pub Manager.

Pub Event Form:

In order to host a dance party in the Lakeland College-Pub, an organization MUST have this form completed one week prior to the event. A Pub Request Form must additionally be filled out at least two weeks prior to the proposed event. Please submit this form to Talia Proffitt, Director of Student Activities in Laun 100 or Sally Bork, Assistant Director of Student Activities in the Campus Center.
